Enrollee Eligibility Requirements

Employee or Spouse/Same Sex Domestic Partner (SSDP) who is enrolled
in a Microsoft medical plan and meets one or both of the following criteria:

1. Diagnosed as obese (BMI greater than or equal to 30);

2. Overweight with a BMI greater than or equal to 27 and diagnosed with
two or more of the following co-morbid conditions:
Congestive Heart Failure
Coronary Heart Disease
Diabetes
Hyperlipidemia
Hypertension
Depression

Weight Management Program Requirements

Intensive Segment:
Month One:
Initial evaluation with Dr. Emery
Weekly one-hour sessions with Dr. Emery
Cardiovascular training session with Dr. Emery
3 one-hour personal training sessions per week
Unlimited use of cardiovascular equipment
You will be given a heart rate monitor to keep
You will be given a digital food scale to keep
Month Two:
Weekly one-hour sessions with Dr. Emery
3 one-hour personal training sessions per week
Unlimited use of cardiovascular equipment
Month Three:
Weekly one-hour sessions with Dr. Emery
3 one-hour personal training sessions per week
Unlimited use of cardiovascular equipment

Maintenance Segment:
Month Four:
Weekly half-hour sessions with Dr. Emery
3 one-hour personal training sessions per week
Unlimited use of cardiovascular equipment
Month Five:
Weekly half-hour sessions with Dr. Emery
3 one-hour personal training sessions per week
Unlimited use of cardiovascular equipment
Month Six:
Weekly half-hour sessions with Dr. Emery
3 one-hour personal training sessions per week
Unlimited use of cardiovascular equipment

Pre-certification Requirement

An eligible enrollee’s regular physician must pre-certify their patient’s eligibility with
confirmation of diagnosis of obesity (Body Mass Index (BMI) ≥ 30) or overweight with a
BMI 27 or greater and diagnosis of two of the co-morbid conditions listed below. Enrollee’s
regular physician must complete a certification form and fax it to the Plan’s Care Facilitation
department. Enrollee must present the certification form to the Provider during their first
visit. Provider may contact Plan’s Care Facilitation department to confirm the enrollee’s
eligibility and pre-certification.

Emery Behavioral Medicine and Microsoft Corporation created their partnership in
2003. In a press release Dr. Emery stated, "The EMERY approach to weight loss
and weight management resonates with Microsoft's goal of providing employees a
level of benefit care that is unmatched in the world."

The Microsoft Weight Management Program has expanded to include over 15 facilities that

The benefit is available to Microsoft employees and their spouses or same sex domestic
partner. Microsoft will pay this benefit only one time, but will pay up to $6,000. This is a
significant incentive, for those employees who are serious about making a change in their life
toward better health. There are some requirements in order to qualify for this benefit.
